Ramadan Exposure In utero and Child Mortality in Burkina Faso: Analysis of a Population-Based Cohort Including 41,025 Children

Abstract

Ramadan exposure in utero can be regarded as a natural experiment to study how nutritional conditions in utero influence susceptibility to disease later in life. We analyzed data from rural Burkina Faso on 41,025 children born between 1993 and 2012, of which 25,093 to Muslim mothers. Ramadan exposure was assigned based on overlap between Ramadan dates and gestation, creating seven exclusive categories. We used proportional hazards regression with difference-in-differences to estimate the association between Ramadan exposure at different gestational ages and under-five mortality. Under-five mortality was 32 deaths per 1,000 child-years. Under-five mortality among Muslims was 15% higher than among non-Muslims (P < 0.001). In the difference-in-differences analysis, Ramadan occurrence during conception, first, or second trimester was associated with higher under-five mortality rates, only among Muslims. The mortality rates of children born to Muslim mothers were 33%, 29% and 22% higher when Ramadan occurred during conception, first and second trimester, respectively, than among children of non-Muslim mothers born at the same time (P = 0.01, P < 0.001 and P = 0.007). Having a Muslim mother was not associated with mortality when not exposed to Ramadan, born during Ramadan or exposed during the third trimester. Observance of Ramadan during early pregnancy can have detrimental consequences for the future health of the unborn child.

The paper presents a versatile solution facilitating calibration of stereoscopic camera rigs for 3D cinematography and machine vision. Manual calibration of the rig and the camera can easily take several hours. The proposed device eases this process by providing the camera operator with several predefined analyses of the images from the cameras. The Image Analyzer is a compact stand-alone device designed for portable 19″ racks. Almost all video processing is performed on a modern Xilinx FPGA. It is supported by an ARM computer which provides control and video streaming over the Ethernet. The article presents its hardware, firmware and software architectures. The main focus is put on the image processing system implemented in the FPGA.

The Epidemiology, Evolution, and Treatment of KPC-Producing Organisms

Abstract

Purpose of Review

The purpose of this review is to investigate the evolution and epidemiology of Klebsiella pneumoniae carbapenemase (KPC)-producing organisms and the current and future treatment options for infections caused by KPC-producing isolates.

Recent Findings

The emergence of resistance in Enterobacteriaceae producing carbapenemases globally has increased the challenges in treating infections caused by these organisms. One of the prominent mechanisms of resistance is the production of KPC enzymes. Infections caused by organisms producing KPCs have limited treatment options and are associated with poor clinical outcomes. The rapid rise of KPC-producing organisms necessitated the use of drugs with pharmacokinetic and toxicity limitations, including polymyxins, tigecycline, fosfomycin, and aminoglycosides. The availability of new beta-lactamase inhibitor combinations that are effective against KPC-producing organisms represent an advance in safety and efficacy. Several agents are currently being studied that have activity against KPC-producing organisms and appear to represent promising additions to our armamentarium.

Summary

KPC-producing organisms cause infections with high morbidity and mortality. Limited treatment options are available, though new therapies have been developed. Pipeline agents are likely to have a place in therapy for the treatment of infections caused by KPC-producing isolates.

The Epidemiology, Evolution, and Treatment of KPC-Producing Organisms

Does the association of blood-derived growth factors to nanostructured carbonated hydroxyapatite contributes to the maxillary sinus floor elevation? A randomized clinical trial

Abstract

Objective

The combination of calcium phosphate with blood-derived growth factors (BDGF) has been widely used in bone regeneration procedures although its benefits are still unclear. The purpose of this study was to evaluate whether or not BDGF improves the efficacy of a modified carbonated calcium phosphate biomaterial in sinus floor augmentation.

Material and methods

Ten patients underwent 20 sinus floor augmentation procedures using nanostructured carbonated hydroxyapatite (cHA) microspheres alone or associated with BDGF in a randomized controlled clinical trial. The in vitro release of growth factors was assessed by an elution assay. Bone grafts were randomly implanted in the right and left maxillary sinuses of each participant, associated either with a 0.9% saline solution or BDGF. Bone gain was evaluated through cone beam tomography after 180 days.

Results

Nine women and one man composed the sample. The blood-derived concentrates were able to release high levels of growth factors and cytokines. A significant clinical advantage was observed in the use of the BDGF after fibrin polymerization around the biomaterial microspheres, optimizing the surgical procedures, thereby reducing the time and displacement, and improving the adaptation of the biomaterial in the maxillary sinus. No synergistic effect was observed in bone formation when cHA was associated with BDGF (p > 0.05).

Conclusion

Equivalent new bone formation was observed for cHA in the presence or absence of the BDGF concentrate in bilateral sinus floor elevation after 6 months. Blood-derived growth factors did not improve bone repair when associated with calcium phosphate in sinus lift procedures.
